Defensive Medicine: Who's to blame?

Defensive practice is constantly on the rise. A punitive approach to error in hospitals and GP surgeries encourages the phenomenon of defensive medicine and acts as an obstacle to the detection and reporting of error. Bills paid by the NHS (and other health systems) are increasing every year. It's time to step back and discuss the root causes of the problem and draw a line between honest mistakes and cross negligence and malpractice.
